Brian Eley, Alan Sive, Margaret Shuttleworth, Gregory Hussey; A prospective, cross-sectional study of anaemia and peripheral iron status in antiretroviral naive, HIV-1 infected children in Cape Town, South Africa: (in English) AbstractBackgroundAnaemia is a common manifestation of paediatric HIV infection. Although there are many causes, anaemia of chronic diseases is the most frequent type. In poor countries iron deficiency is widespread. It is probable that many HIV-infected children in these countries are also iron deficient. This study describes the relationship between paediatric HIV infection and anaemia, and documents the peripheral iron status of antiretroviral naive, HIV-infected children.MethodsSixty children were evaluated prospectively. Investigations included CD4+ count, haemoglobin concentration (Hb), red blood cell (RBC) morphology, and iron studies.ResultsAnaemia was present in 73% of children. Compared to mild HIV infection, median Hb was lower in children with moderate clinical infection (104 g/L v 112 g/L, p = 0.04) and severe clinical infection (96 g/L v 112 g/L, p = 0.006), and more children with severe infection were anaemic (92% v 58%, 0.04). There was a significant relationship between immunological status and Hb. 68% had abnormal RBC morphology. Significantly more children with moderate and severe disease, and severe immunosuppression had abnormal RBC morphology. 52% were iron-depleted, 20% had iron-deficient erythropoiesis and 18% iron deficiency anaemia (IDA). 16% (7/44) of anaemic children had microcytosis and hypochromia. Median soluble transferrin receptor concentration was significantly higher in those with microcytic hypochromic anaemia (42.0 nmol/L v 30.0 nmol/L, p = 0.008).ConclusionsBoth the proportion of anaemic children and the median Hb were associated with disease status.   1. ... , Chan, Ida ... ; Aspect-Oriented Incremental Customization of Middleware Services: As distributed applications evolve, incremental customization of middleware services is often required; these customizations should be unpluggable, modular, and efficient. This is difficult to achieve because the customizations depend on both application-specific needs and the services provided. Although middleware allows programmers to separate application-specific functionality from lower-level details, traditional methods of customization do not allow efficient modularization. Currently, making even minor changes to customize middleware is complicated by the lack of locality. Programmers may have to compromise between the two extremes: to interpose a simple, well-localized layer of functionality between the application and middleware, or to make a large number of small, poorly localized, invasive changes to all execution points which interact with middleware services. Although the invasive approach allows a more efficient customization, it is harder to ensure consistency, more tedious to implement, and exceedingly difficult to unplug. Thus, a common approach is to add an extra layer for systemic concerns such as robustness, caching, filtering, and security. Aspect-oriented programming (AOP) offers a potential alternative between the interposition and invasive approaches by providing modular support for the implementation of crosscutting concerns. AOP enables the implementation of efficient customizations in a structured and unpluggable manner. We demonstrate this approach by comparing traditional and AOP customizations of fault tolerance in a distributed file system model, JNFS. Our results show that using AOP can reduce the amount of invasive code to almost zero, improve efficiency by leveraging the existing application behaviour, and facilitate incremental customization and extension of middleware services. Roberson, David Warren; Automated Second Echelon Commitments for the C3EVAL Model: The C3EVAL model is being developed by the Institute for Defense Analyses (IDA) for the Joint Chiefs of Staff (JCS/J6). The model is to provide one element of a workstation being developed for JCS that is to be used to evaluate theater level command, control and communications (C3) in terms of combat consequences. The red commander enhancement to the C3EVAL model provides a red side that is independent of the blue side. The enhancement also automates the commitment of second echelon forces on the red side. Parks; (in English) DETERMINING STAFF DEVELOPMENT NEEDS TO ACHIEVE COMPUTER TECHNOLOGY COMPETENCE BY INSTRUCTIONAL PERSONNEL IN GLOUCESTER COUNTY PUBLIC SCHOOLS By James Larry Hoover David J. Parks, Chairman Educational Leadership and Policy Studies (ABSTRACT) The Virginia Board of Education is considering the adoption of technology standards for instructional personnel in Virginia's schools. All pre-service instructional personnel will have to demonstrate proficiency prior to obtaining a Virginia teaching certificate. Those currently in service may also have to demonstrate proficiency, as defined by the local school division. This study was designed to be a prototypic model for implementing the standards in Gloucester County Schools. Three hundred-ninety (390) instructional personnel were surveyed, according to selected strata, regarding their current level of competence. The discrepancy between the current level of proficiency and the level required by the standard defined the need for staff development. Differences among strata were provided for in planning for staff development activities. Staff Development activities were planned for all identified needs and implemented for a pilot group. The training sessions focused on raising the level of proficiency to that required to satisfy the standards. Participants in the pilot group were re-surveyed to verify the plan. An assessment procedure for measuring and reporting mastery on the standards was proposed by the pilot group. The process outlined in this study will be condensed into a brochure that will be provided to the Virginia Department of Education for possible distribution to other school divisions.
